Participant take-aways after 9 years of PHT funded Mindfulness Courses in Tasmania

This survey is in 4 parts: Consent, participant information, three research questionnaires (mental health, wellbeing and mindfulness) and the opportunity for you to write your thoughts and experiences in response to some open-ended questions. It will take about 15 minutes to complete.

Part 1. Consent

Have you read and understood the Participant Information Sheet for this Research Project?

Do you consent to your anonymous data being included in this Research Project, and can you agree to the following:

  1. I understand that the purpose of this research project is collect survey data from Tasmanians who have completed a mindfulness course with Mindfulness Programs Australasia

  2. I understand I will be invited to complete a survey that asks questions about my mindfulness experience

  3. I understand that the researcher(s) will maintain confidentiality and that any information I supply to the researcher(s) will be anonymous and only used for the purposes of this research. 

  4. I understand it will not be possible to identify me in any reports or publications arising from this research.

  5. I have read the Participant Information Sheet and am aware of the potential risks of participating in this study. 

  6. I understand that my participation is voluntary and I am free to withdraw from the project at any stage, without explanation or penalty. 

  7. I understand that all research data will be securely stored in digital form on the MPA premises and may be re-used in future related studies investigating further aspects of mindfulness experience

  8. I agree to participate in this study.
